how do i launch a formal complaint? "un-P related"

Swear, if i get a flare up from all this stress, im going to be REALLY ANGRY!!!
I recently ordered dsl from at&t worldnet, and before i agreed to order from them, i asked about the cancellation process. THey told me, all i had to do was stick the self label back on, and send the kit back if i didnt want it for any reason. Turns out my computer was too old, so i did all that. 2 months later, i got a bill for $191.00.
When i called, they initially told me they didnt recieve the DSL kit, thats what the charge is for, i gave them the tracking number, and the perosn i spoke too said her supervisor was unavailable. But would call me within 2 days. No one called me.
I didnt pay teh charge and today, i got another bill, same charge, so i call again. Explained what happened, now the story changes. They recieved the DSL kit but the charge is for cancellation. I wasnt told to formally cancel, all i was told to do was send the kit back. The rep refuse to help me, so i asked to the supervisor.
After listening to my story, the sup. still refused to help me. I know im not wrong for this guys, they're asking me to pay for something i didnt know about. They noted that thier sales person MADE A MISTAKE, TOLD ME THE WRONG INFORMATION. After establishing all that, they still refuse to take the $191 off the bill, and still hold me responsible
ooomg! im already stressing as i write this.
The supervisor then made it clear it was MY FAULT for believing the sales rep, and i should've read the terms agreement. Now, i ordered this kit ON THE PHONE, there was NO VISIBLE agreement to be read, yet this person still blames ME!!! Refuses to help me. basically paraphrasing "sorry, its not your fault, but you stil have to pay"
By this time, im totally angry. This person starts giving me the run around, so i said "its obvious you cant show me the respect to speak to me in a civil manner, let me have your name and ID number so my father can deal with this"
She gave me her name but refuse to give me her ID number.. saying i didnt need it. WTH!!!!!! THis made me angry, after i went off about her not giving me proper information, she says "ooh, hehe, sorry, i didnt hear what you just said" Then i go off about her lack of perfessional attitude. I AM THE CUSTOMER! It doesnt MATTER if you dont like what i had to say, the fact is, you are there to HELP ME. NOt tell me im shiet out of luck for something I DIDNT KNOW!!!
she then counters with "well what about your professional attitude" ooomg!!! helllooo!! i am the customer, i dont need to be professional, YOU DO!!!
Anyways, after about an hour of this, of her STILL REFUSING TO GIVE ME HER ID NUMBER AND EXTENTION (friggin supevisor) ... she did the one thing that just BLEW ME UP.... SHE HUNG UP ON ME!
YES! while i was mid sentence, she GRUNTED then HUNG UP ON ME! Hello!! THis is a supervisor at the BILLINGS DEPARTMENT of AT&T Worldnet!!
Im sooo angry right now you guys. My question is.. HOW DO I LAUNCH A FORMAL COMPLANT?? I keep getting the run around when i try, its pissing me off, making me stress out. UGH!

First begin a paper trail with a formal letter outlining your position and send it to them. Ask for the solution you want, then wait a week.
If you do not get a positive response from them, send a letter outlining your complaint and attempts to recify the situation along with a copy of the letter you initially sent to the Better Business Bureau. You may want to send one along the your State's Atty General as well.
