If consumers wish to replace discs 5 and 6, which are the only ones affected, they can write to the following address free of charge, including their original ones:

So I just send my discs off to the address? What happens if they go missing?

Either send them "recorded" delivery or simply get a cert of posting - at least you can claim then.
Alternatively you could simply return the whole set to where you bought it, demanding an exchange for the ("we-haven't-really-but-we-actually-have") re-issued set! What you have is essentially a "faulty" product and there's no reason at all why you can't get it replaced by the retailer.
