Why does my Sky and dvd (both rgb) look slightly grainier when i remove my terrestrial aerial?

Cos you're getting bleed through of the RF signal, and when its tuned its less grainy than the dance of the untuned bumblebees?
Yep, that would do it.
Solve the problem with a better quality SCART lead, or, better still, one that is wired for one direction only. If that doesn't do it, junk the TV.

if i get a good quality lead will the rf breakthru problem disappear? e.g. pic quality will be the same whether rf is connected or not? and hopefully my patterning prob should go also?

It may do - there really is only one way to try it and that's get the lead to try it. If it does then great, if it doesn't then the TV should be going back IMO.
