Can anyone help ?
I have been set a task on my ITEC Beauty Level 2 course of putting together a consultation card for manicures (natural nails). I'm at a loss as to what needs to go on there. Does anyone have any ideas ?

Basically you need their name, address, contact number, gp details, lifestyle/hobbies, a list of contra indications, allergies they can tick off, an area for notes for you to write any comments about their nails, the shape they like, any problems, any treatments you recommend, what you did that particular day, colour etc, a place for them and you to sign and a disclaimer basically saying they have told you the truth and are happy for the treatment to commence. You should then check to make sure nothing has changed on the following appointment, and note your treatment for that day.
Also if you recommend that you cannot treat them for whatever reason, maybe fungal infection or you feel they need to be referred to gp, stick to your guns, and also make comments on the sheet they have signed.
Imagine if another therapist were to step into your shoes because you were away, she would need to know all about the client and treatment etc. That's another way of looking at it.
